6/28/2023 EnchantaVaughn Approved Behavior: (AM) Upon arrival, client was observed in class displaying age appropriate social skills. Client was able to demonstrate good listening skills by paying attention during each activity. Client was very engaged into his classwork. Client was witnessed in his seat and on task. Client utilized positive reinforcers that was provided by TDT staff. Client appeared to be in a good mood, as evidenced by his happy and content expressions. Client walked quietly to lunch, showing polite and respectful attitude toward peers and staff. (PM) Client was monitored and supported for being a team player and helping others clean up and throw away their trash. Client is seen working independently and completing all assignments on his own. Client exhibits moderate level of participation in class. Client was involved in appropriate social interaction with his peers. Client is observed transitioning to and from class in a positive manner. Client ignored mental health professional and expressed non verbal communication by rolling his eyes and smacking his lips. (EOD) Client presented himself in a rude and disrespectful manner toward QMHP. Client displayed a negative attitude in psycho social setting. The client appears to be functioning poorly in his crisis situations due to refusing support and supervision that he needs. Intervention: (AM) QMHP had one on one with client. QMHP reviewed yesterday's activities. QMHP provided support and encouragement to ensure goals and objectives are met. QMHP monitored client individualized behaviors in collaboration with his teacher and social environment. QMHP encouraged client to make good choices and to avoid negative thoughts about himself. QMHP processed client avoidant behaviors. (PM) QMHP assisted client in developing more social skills. QMHP inquired about possible challenges client can experience with applying this skill. QMHP provided positive feedback. QMHP reflected on client goals and objectives. QMHP isolated client from distractions to decrease disruptive behaviors and to reduce episodes. QMHP assisted client in normalizing the changes that are occurring in school. (EOD) QMHP explored the clientâs capability to cope with his stressors and to get back on track. QMHP modeled the correct behaviors and conduct. QMHP assisted client with finding and utilizing necessary resources. QMHP verbalized the awareness that all people are unique, learn differently, and have various strengths and weaknesses. Response: (AM) Client reflected on weekend activities. Client did not respond to QMHP inquiries and chose to walk away during his session. Client was not acceptive to support and encouragement. Client expressed negative thoughts about having counseling and was giving off bad energy. Client was seen making poor choices and disregarding positive reinforcers. Client communicated he wanted to work with male counselor and that his mom told him to talk to administrative. (PM) Client was displaying good social skills by interacting well with peers. Client is still learning to build relationships and contribute to conversation. Objectives were partially met at this time, due to at home stressors. Client did not want to disclose any information and stayed to himself. Client ignored interventions and was having a difficult time processing his feelings with QMHP. Client expressed negative feelings about changes occurring at school. (EOD) Client was not engaged in reflective thinking. Client communicated the do's and don'ts. Client was able to comply with authority rules and regulations once family member showed up. Client utilizes help and support when he wants. Client communicates and displays undesirable traits at school and is learning to cope with his crisis situation with family support.
